Onflo (formerly Let's Talk!) Form Builder is a convenient way to launch a new Onflo form for any department or topic, including bullying, safety, transportation, human resources, and more! 

This feature allows you to quickly create new forms so you can capture the critical information needed to provide faster, superior service to your customers. 

To get started, navigate to Settings>Forms

Select “Add Form” to create a new form or choose a form from an existing pre-built template such as “Report Bullying” or “General Questions or Concerns.”

From there, you can start building your form. First, select whether you would like to use one of the following form types:

Screenshot 2026 01 08 181403Default Form: Basic form that provides standard fields including the following:

  • Question Type
  • Message/Description
  • Customer Type
  • Customer Contact Information

Standard: Provides default form fields plus additional fields including the following:

  • Ticket Type
  • Ticket Subject
  • Ticket Description
  • Customer Contact Information
    • Customer Type
    • Name
    • Email
    • Phone Number

IT Form (ITSM/ITAM Customers only): Used for providing Support requests for specific devices related to hardware, software, or to identify system issues.

Once you’ve opened your new form, add your form name and select the topic. You can apply the same form to multiple topics.

Form Menu

Our Form Menu allows you to do the following:

  • Assign form to a topic
  • Import questions from existing form
  • Default Values (questions pre-populated on form)
  • Add Questions (by type)
  • Add Form Element (text and image)
  • Allow Hidden Submissions (toggle on/off)

Throughout the form, you’ll see standard questions already populated. If you’d like to hide any questions, you can choose to hide the question, which will remove it. To do so, click on the question, then click the Hide Question toggle.

Next, add questions. To add a new question, select “Add Question”. The new question will appear under the preceding question.

From there, you can click on the “Question” text in the box to customize your question

To select the question type, navigate to the dropdown menu on the right and select one of the options.

Form Builder offers the following supported question types:

  1. Short Text (with validation)
    1. Number
    2. Phone Number
    3. Phone Number with Extension
    4. Email
    5. URL
    6. Character count (Min./Max.)
  2. Long Text
  3. Multiple Choice (single-select)
  4. Checkboxes (multi-select)
  5. Dropdown (single-select)
  6. Add Attachment
  7. Date
  8. Time

Additional Question Tools 

In addition to selecting your type of question you can access additional features by clicking the three vertical dots icons for questions:

You can choose from the following options:

  • Add Help Text
  • Show question based on answer
  • Show ticket rules (change topic or add tags)
  • Disable form submission based on answer. You can choose which question(s) to be disabled.

Note: Selecting the "Disable form" option will prevent a form from being submitted based on selections to questions. This may be especially useful if you wish for customers to be redirected to another department or resource for assistance.

Required Questions

You can choose to make questions required by toggling the switch on in the lower right corner of the question box. You can also duplicate or delete the question.

Continue following these steps until your form is complete. You can reorder questions as needed by using the drag-and-drop feature. You can also add logic to your forms or translate questions into different languages to streamline the user experience and use one form to serve multiple stakeholders (like students and parents). 

Preview and Save Form

To preview your form before you publish it, select “Preview” in the lower right corner of the Form Builder.

While working on your form, you can select "Save Progress" while you continue to work on the form. If your work on the form is complete and you believe the form is ready you can click Save and Exit.” to leave the Form Builder tool.
